When did you last have someone go through all of your medicines with you? Not just check a prescription but really go through them: what each one does, whether you still need all of them, whether they’re working well, and whether they could be behind symptoms you’ve been putting up with.

What is a Home Medicines Review?

An accredited clinical pharmacist visits you at home and reviews everything you take together: prescriptions, over-the-counter products, vitamins, supplements, patches, inhalers, injections and eye drops.

How do they work?

A visit at home
Relaxed and private, with your actual medicines in front of you, not a list on a screen.

A report for your GP
Written findings and personalised recommendations, sent straight to your doctor.

A plan you agree on
Your GP talks the report through with you and you decide on any changes together.

What happens, step by step

Your GP refers you

It takes them a minute. Nick, the pharmacist, then phones you to arrange a time that suits. A family member or carer is welcome to be there.

The home visit

You go through everything together, without time pressure. Ask anything you’ve been meaning to ask.

Report & follow-up

Your GP receives the report and talks it over with you at your next appointment. Up to two follow-up visits with Nick are available if they’re useful.
