Coordinated Veterans’ Care Program

Personalised, ongoing healtcare for eligible DVA card holders

The DVA Coordinated Veterans’ Care (CVC) Program provides eligible veterans with personalised, ongoing support to manage chronic health conditions. At Grove Medical Cockburn, your regular GP and practice nurse work together to coordinate your care, develop a personalised plan based on your health goals and provide regular follow-up. The aim is to keep you well, identify concerns early and reduce the risk of unplanned hospital visits.

Who is Eligible?

You may be eligible for the CVC Program if you:

  • hold a Veteran Gold Card and have one or more chronic health conditions; or
  • hold a Veteran White Card and have a DVA-accepted chronic mental health condition;
  • live in the community; and
  • have complex care needs and are at risk of an unplanned hospital admission.

Your GP will review your individual circumstances and confirm whether you meet the full DVA eligibility criteria.

Is there a cost?

The CVC Program is funded by the Department of Veterans’ Affairs, so eligible participants will not have an out-of-pocket cost for appointments provided as part of the program.

Other healthcare services remain subject to DVA eligibility and acceptance requirements. We will let you know before providing any service that may involve a fee.

How to Get Started

New to Grove Medical Cockburn?

Book a new patient appointment with one of our Cockburn GPs to discuss the program and confirm whether you meet the DVA eligibility criteria.

Please bring your Veteran Gold or White Card, a current list of your medications and details of any specialists or allied health providers involved in your care. If available, please also bring any recent test results, hospital discharge summaries or referral letters.

Already a Grove Medical Cockburn patient?

Please call our Cockburn team on (08) 6285 8190. We will ask you a few questions and review your medical record to determine whether you can book directly into a CVC care plan appointment. If further assessment is needed, we will arrange an appointment with your GP first.

What happens next?

Once your eligibility has been confirmed, you will attend a 60-minute appointment with our practice nurse to establish your personalised care plan. This will be followed by a 15-minute consultation with your regular GP to review the plan and complete your enrolment.

What to expect from your ongoing care?

Once enrolled, your regular GP and practice nurse will work together to support you and coordinate your care.

  • Monthly phone check-ins:  your practice nurse will call you each month to discuss how you are going, follow up on your care plan and identify any concerns.
  • Three-monthly reviews: you will attend an in-person appointment every three months to review your health, progress and care plan.
  • Care coordination: your practice nurse can help coordinate referrals, appointments and communication with the healthcare providers involved in your care.

The program provides proactive, ongoing support rather than a one-off appointment. You can remain enrolled for as long as you continue to meet the eligibility criteria and benefit from the program.

Your GP will review your Veteran Card, health conditions and care needs against the DVA eligibility criteria. Meeting the basic criteria does not automatically guarantee eligibility for the program.

No. The CVC Program is funded by the Department of Veterans’ Affairs, so there is no out-of-pocket cost for eligible participants.

The CVC Program must be delivered by your usual GP. If you are new to Grove Medical Cockburn, you will first need to book a new patient appointment so that your GP can understand your health history and assess your eligibility.

Existing Grove Medical Cockburn patients may be able to book directly after our team reviews their medical record and confirms their likely eligibility. If further assessment is required, we will arrange an appointment with your GP first.

Your initial assessment, enrolment and three-monthly care plan reviews must be completed in person at Grove Medical Cockburn. Your monthly check-ins with the practice nurse can be completed by phone.

Your care plan outlines your health goals, treatment needs and the support required to manage your ongoing care. It may also include referrals, follow-up arrangements and coordination with other healthcare providers.

Your regular GP oversees your care plan and works closely with our practice nurse. The practice nurse maintains regular contact with you and helps coordinate the services included in your plan.

Yes. With your consent, a family member, carer or support person can attend appointments and be involved in discussions about your care.

Our practice nurse will generally contact you each month, and you will attend an in-person review with your care team every three months.

There is no fixed end date. You can remain enrolled while you continue to meet the eligibility criteria and benefit from the program. Your eligibility and care plan will be reviewed every three months.

Yes. Participation is voluntary, and you can withdraw at any time. Speak with your GP or practice nurse if you would like to leave the program.
